General Referral Guidelines:

• Download our referral form, fill out completely, and fax to Erica Hastings at 617.414.4796           

• Print a map  of BMC  to assist your client in navigating our large campus.

• Keep in mind that it often takes several weeks to book an appointment for new patients.

• Non-torture survivors may be referred to other providers within Boston Medical Center.

If you are requesting medical and/or psychological evaluations for affidavits:

• Make the appropriate referrals at least 3 months prior to when the affidavit is needed.

• Specify type of affidavits needed (psychological, medical, and gynecological) based on your interactions with the client.

If you need a medical release form:

• Print a release form,  fill out completely, and fax to 617.414.4210.
